If your behavior does not appear impaired, but you admit that you have used drugs within a certain period before your arrival in the United States, the doctor may determine that you have been "abusing" drugs. This could lead them to order a drug screening.
However, if you want to immigrate to the United States of America, it is a good idea to be honest with yourself about your drug use and make sure that you don't need medical help.

You must pass the immigration physical test if you want to become a permanent resident of the United States. One of the components of this test could be a drug screening. If you have used illegal drugs in the past six months, you will need to provide proof that you have completed an approved drug rehabilitation program.
To answer your question, the immigration physical test does not include a drug test. If you are selected for a drug test during the visa application process, that is done later. These tests are regulated by the US Department of State (DOS).
So, if you are chasing the infamous “American Dream” make sure you are aware that you could be ordered to pass a drug test.
