Immigration physicals are required by law for all non-citizens immigrating to the United States.

A physical exam must meet certain requirements: it must be completed by an authorized medical professional, it must be done within one year preceding the date of application, and it must include tests for tuberculosis (TB), as well as blood tests for syphilis and gonorrhea. Depending on the country from which you're coming, there may be additional requirements for vaccinations.
